Get an intensive, multi-faceted orientation to the science of change in primary care at the June session of Practice Coaching for Primary Care Transformation training. This four-day course equips you with practical tools and coaching skills to engage and support your clinic team to evolve into a high-performing patient-centered medical home. Optional follow-up support is adapted to the group’s needs.
The course was created jointly and is taught by the UCSF Center for Excellence in Primary Care and CareOregon’s Patient & Population Centered Primary Care Program.
