This page contains information for currently enrolled members and potential members of the Oregon Health Plan. If you don't find answers to your OHP questions in the information below, you can always contact OHP Customer Service for assistance, or visit the Oregon Health Plan website

Oregon Health Plan Customer Service

There are two ways to contact OHP for questions or to make updates to your current information:

1. By telephone (toll-free): 1-800-699-9075

This is the preferred method if you have a question. Existing members can call OHP for any of the following:

  • to change your address, phone number, family status, CCO or other information
  • to replace a lost OHP Card
  • to check the status of your application
  • to see if you or your children are still covered by OHP
  • to change plans
  • to solve a problem or make a complaint
  • to get an OHP Client Handbook

2. By e-mail: oregonhealthplan.changes@state.or.us


Existing members can e-mail OHP: 

  • to change your address, phone number, family status, CCO or other information

 

PLEASE NOTE: the e-mail address is for changes only. When you send your email to OHP, you must include your full name, date of birth, medical ID number and phone number.
