What is Connect to Care?

Medicaid (or OHP) can be complicated, but we’re here to help make it easier. How do we do that? With Connect to Care, CareOregon’s community outreach team.

The Connect to Care team can provide one-on-one assistance to our members, potential members and community partners. We offer a variety of social supports and give practical one-on-one guidance in navigating OHP (including access to oral language help) and other services. Here are some of the health and social services the Connect to Care team can help with, whether in-person or over a screen:

  • Connect members with complex health needs to our Care Coordination team.
  • Order replacement Member ID cards.
  • Check eligibility for Medicaid and Medicare.
  • Help with OHP enrollment.
  • Help members update their records (address, phone number) with OHP.
  • Help make appointments for physical, dental and mental health care.
  • Connect individuals to Housecall Providers for palliative and hospice care.
  • Explain extra benefits such as language interpreter services, prescription drugs, and support for pregnant members and young children.
  • Arrange nonemergent medical transportation (NEMT) and night stays at shelters, if needed.
  • Assist with enrolling in SNAP (Supplemental Nutrition Assistance Program) for an Oregon Trail Card (food stamps).
  • Discuss options for prepaid phones and other tech needs.

When it's safe, we'll re-evaluate returning to in-person events, working with partners in Clackamas, Columbia, Multnomah and Washington counties. We hope to return to locations such as food banks, health fairs, low-income housing, shelters, college campuses, churches and  the CareOregon building.
