Living Well With Chronic Conditions is a fun, 6-week interactive workshop where you learn practical real life skills to manage a chronic condition. Participants share their successes, develop relationships with others participants which creates an atmosphere of support. The workshop builds confidence around managing your health, staying active and enjoying life.

Workshop Description

Program Length: A 6-week workshop that meets 2 ½ hours each week.

Who should attend: Anyone living with chronic health conditions, including diabetes, arthritis, asthma and heart disease. Family members and friends are also welcome.

How will the Workshop help me?: You will learn skills to actively manage your chronic condition, giving you a greater sense of control over your life.

Who will lead the Workshop?: Two trained leaders. One or both of the leaders has a chronic condition.

Cost: Some Workshops sites may charge a small fee. CareOregon will reimburse the cost of the Workshop when you attend at least four of the six Workshop sessions. To receive your reimbursement:

  • Complete the Workshop Reimbursement Form 
  • Have one of the Workshop Leaders sign the form
