Housecall Providers is a nonprofit health care organization that brings medical care to people in their homes. For almost 30 years, they have treated patients who are homebound or seriously ill, very sick or can’t leave their homes.
Housecall Providers is part of the CareOregon family.
The Advanced Illness Care program
The Advanced Illness Care program helps people who have a serious illness. It provides special care to make them feel more comfortable and less stressed.
If you qualify, Housecall Providers can help you by phone or by visiting you at home. This care is free for patients who are eligible.
If you join the program, the staff will work closely with you and your other health care providers.
They will listen to what matters most to you. Their goal is to help you live as well as possible with your illness and improve your quality of life.
The Advanced Illness Care team
The team includes:
- Nurses
- Social workers
- Community health workers
- A spiritual counselor
Support offered
- Pain and symptom management
- Expert answers about your illness, medicines and treatment options
- Counseling and emotional support
- Communication with your primary care provider (PCP) and specialists
- Care to help you avoid emergency room visits
- Other resources to support you and your care
Extra support for advanced illness
The Housecall Providers Advanced Illness Care program gives you and your caregivers extra support. It will help you live as well and as comfortably as possible with your illness.
Examples of advanced illness:
- Cancer
- Heart disease
- Liver or kidney disease
- Lung conditions such as COPD
- Neurological diseases (affecting the nervous system) such as Parkinson’s or ALS
- Immune diseases such as HIV/AIDS
