Living with chronic conditions

Chronic conditions are health problems that last a long time and usually don’t go away completely. But you can still live well with these conditions by learning how to manage them. Classes, workshops, and programs can teach you helpful skills, reduce your symptoms, lower stress, and give you support. These programs are offered online and in person across Oregon. 

What is considered a “chronic condition?” 

Types of chronic conditions include: 

  • Diabetes or prediabetes
  • High blood pressure
  • Heart disease
  • Stroke
  • Arthritis
  • HIV/AIDS
  • Chronic pain
  • Cancer

Chronic disease self management program  

These programs offer hands-on workshops that teach adults how to manage their health and improve their daily life. Participants can share their experiences and support each other. The workshops help build confidence, keep you active, and help you enjoy life. Most programs are free.

What is covered in a self-management workshop?

Topics covered:

  • How to manage symptoms
  • How to take your medications
  • How to work with your health care team
  • How to set weekly goals
  • How to solve problems
  • How to communicate better
  • Ways to relax
  • How to handle tough emotions
  • Tips for healthy eating
  • Safe and simple exercises

Chronic disease self-management workshop description

Length: The workshop runs for six weeks and meets for 2½ hours each week. 

Who should attend: Anyone living with a chronic condition such as diabetes, arthritis, asthma, or heart disease. Family and friends are welcome too. 

How it helps: You’ll learn skills that help you take control of your health and feel more confident. 

Who leads it: Two trained leaders guide the workshop. At least one leader also has a chronic condition.

Cost: Most workshops are free.
