OHP Provider Enrollment Agreement Notice

Beginning January 1, 2021, the Oregon Health Authority has added a new required element for providers seeing CCO members. In addition to the data collected for the CCO to enroll a provider with OHA, a signed provider enrollment agreement is required for all new practitioner and organization enrollments.

If you are already actively enrolled with OHA, there is no action needed at this time. However, if you are requiring enrollment  or have a new provider requiring enrollment, this agreement must be submitted prior to the provider being eligible to receive Medicaid funds. You can find a stand-alone copy of the newly required form on OHA’s website here, or you may use CareOregon’s practitioner or organizational applications to complete the agreement.
