Supporting access to care for Tribal Communities

 

For years, CareOregon has been honored to partner with the Native American Youth and Family Center (NAYA) and Oregon’s nine federally recognized Tribes to support the health and well-being of Native communities. We’ve worked alongside NAYA to invest in programs that provide culturally responsive care, reinforce self-sovereignty and ensure access to essential health services.

“For more than a decade, CareOregon has been a vital partner in NAYA’s mission to uplift the Native community and improve health and well-being across our region,” said Oscar Arana, CEO, Native American Youth and Family Center. “Their steadfast support for our events and community health programs has enabled us to create meaningful change, provide essential resources and address health disparities—all while strengthening cultural connections and fostering a resilient community. We are deeply grateful for CareOregon’s partnership and shared dedication to building a healthier, more equitable future.”

Another key part of this work is our Tribal Care Coordination program, which helps Tribal members navigate health care, overcome barriers and access the care they need. Our small but dedicated TCC team has built strong relationships with members that ensure they receive personalized, compassionate support. Since 2017, we’ve served thousands of Tribal members and completed more than 1,300 care plans to meet individual health needs.

This commitment reflects CareOregon’s broader mission: to invest in the health of our communities, strengthen partnerships and create lasting, meaningful change in the lives of those we serve.
