New! Submit DME Authorization Requests using CareOregon Connect

We’re excited to announce that you can now submit your DME (Durable Medical Equipment) authorization requests electronically through our online provider portal, CareOregon Connect!

The DME authorization forms currently available online will only be available while we transition to this new workflow. As of October 1, 2024, all DME authorization requests will need to be submitted using CareOregon Connect. Our existing online forms you’re used to faxing today, as well as the fax process, will be discontinued at that time. Please transition to using this new, electronic submission option, right away to ensure a smooth transition for you and your teams.

To access our CareOregon Connect provider portal, simply log in through OneHealthPort. If you do not have a OneHealthPort account, please register your organization today. Visit the OneHealthPort site to register for an account. For questions related to registering for a OneHealthPort account, please contact OneHealthPort at (877) 814-9909, or email them at connect@healthtrio.com.

Here are some things we want you to know:

  • Find our “How to Enter a DME Request” tutorial video and guide on our Provider Portal Tutorials webpage.
  • Remember that medical records must be included with each submission.
  • When completing the form, please ensure you:
    • Enter your DME organization name twice - both as the “Prescribing” and “Servicing” provider.
      • Prescribing providers will be identified using the attached medical records, as needed.
    • Please avoid selecting the general “Durable Medical Equipment” service category if a more specific service category is more appropriate for the service you’re requesting.
    • Be sure to submit all CPT codes separately using the appropriate template option (office and surgical procedures, therapy, etc.)
